Introducción
La deformidad del pie conocida como pie equino varo congénito idiopático es una de las patologías ortopédicas más comunes que se dan en el recién nacido. Dentro del tratamiento conservador se conocen fundamentalmente tres técnicas: Kite, Ponseti y método funcional francés. La correcta utilización de los dos últimos métodos puede evitar la intervención quirúrgica de liberación.
•Método Ponseti: Este método de tratamiento consta de dos fases. La primera fase, consiste en la manipulación suave del pie, el enyesado para mantener la corrección lograda con la manipulación, la repetición de estas acciones cada semana en un promedio de entre 4 y 8 semanas y la realización de la tenotomía del tendón de Aquiles para la corrección del equino con la subsecuente colocación del último yeso en hipercorrección, el cual permanecerá de 2 a 3 semanas a fin de que el tendón de Aquiles cicatrice en su nueva posición. La segunda fase del tratamiento consiste en mantener la corrección lograda con una férula en abducción que deberá utilizarse hasta los 4-5 años de edad, con un protocolo de uso perfectamente estandarizado.
•Método funcional francés: consiste en la manipulación pasiva diaria del pie zambo del recién nacido, la estimulación de los músculos que rodean el pie, para mantener la reducción lograda previamente, y la inmovilización temporal con vendaje funcional. Los tratamientos se realizan durante dos meses, y después se van reduciendo progresivamente durante 6 meses más. A continuación, se sigue con el vendaje, hasta que el niño empiece a caminar. Se utilizara una férula nocturna durante 2-3 años.
Objetivos
Comparar la eficacia de los dos métodos más utilizados en el tratamiento conservador en fisioterapia .
Metodología
•Se realiza una revisión bibliográfica de la siguientes base de datos: PubMed, Dialnet y Scopus.
•Criterios de inclusión: 5 últimos años, inglés y español.
•Términos Mesh: Methods, Physical therapy modalites, clubfoot.
•Se encontraron 6 artículos de los cuales seleccionamos 2 artículos.
Resultados
• Los pacientes con pie equino varo tratado tuvieron puntaje promedio de la función motora gruesa en comparación con compañeros de la misma edad .
• Los pacientes en el grupo del método funcional francés obtuvieron puntuaciones más altas en Velocidad de carrera / Agilidad (P =
0.019), Rango percentil de coordinación corporal (P = 0.038), y rango percentil de fuerza y agilidad (P = 0.007) Que los pacientes
tratados con la técnica de Ponseti.
• Los pacientes con pie equino varo bilateral obtuvieron puntuaciones significativamente más bajas en la sub-prueba de Balance (P
<0.01), y rango percentil de Coordinación del Cuerpo (P <0.01), que aquellos con pie zambo unilateral.
•Los pacientes que requirieron cirugía puntuaron significativamente más bajo en la sub-prueba de Balance (P = 0.04) que los que no
requirieron cirugía.
•Importancia de empezar el tratamiento el primer mes de vida para un pronostico favorable.
Conclusiones
• No podemos concluir con respecto a los datos encontrados que método es más eficaz en cuanto a la corrección del pie equino varo.
• Los resultados nos indican que ambos métodos son eficaz.
• Mejor pronostico pacientes tratados con tratamiento conservador respecto pacientes quirúrgicos.
• Mejor pronostico tratamiento precoz (1er mes de vida).
• Seria interesante valorar estudio de combinar ambos métodos recogiendo sus puntos fuertes .
